    Concerning the Certification of Citizenship/Compliance form.

    Line 1) Name under which the application was filed.
    Is that an individual, trust, dealer ect? In other words would it would be my name as an individual?

    Line 2) Name of person certifiying to citizenship.
    Pretty obvious there, but asking to be sure I don't make any mistakes.

    Is there no proof of citizenship required such as a birth certificate ect?

    I don't know about for a trust, but for an individual you put your name in both fields. You're certifying yourself. It's dumb, I know.

    The citizenship form is NOT needed for a trust or LLC transfer. So under and individual you put yourself, but under anything else you just don't fill that form out at all. :D

    Oh, that $600 for my trust was the best money ever spent. I just fill out forms myself and send them in, no need to involve pictures or LEOs or citizenship forms etc. And I can "share" my trust's NFA items with the other trustees (dad, brother, fiance).
