Sorry if this has already been posted for NW Indiana.
My understanding is that this includes the 65 dollars for the Utah license and 200 of it is to Blythe's. 5 hour class - no range time involved. However for an additional 75 dollars, you can get the Florida non-resident card as well. (although it only adds another 3-4 States over the Utah non Resident.)
Having the Utah Permit adds the following states. (assuming you already have the Indiana LTCH)
Washington Nebraska
Minnesota Wisconsin (Yeah, for state number 49!)
Ohio West Virginia
Virginia Delaware
Not good in the usual culprits... CA, NY,OR,NV,KS, ILLINOIS of course, MD,DC,NJ,MA,RI,ME,SC
