    I have a Beretta SP1 O/U shotgun that has started having an issue recently.

    If I set the top barrel to fire first, the second barrel doesn't always fire when I pull the trigger. It works just fine when I set the bottom barrel to fire first.
    What could the issue be?

    I agree with Dean and Lumpy on this one. Powder residue, grease or oil etc can get in there from around the firing pins and gum stuff up. Might be wise to search around to see if anything internal should be lubed. Many O/U's it's best not to lube the firing pins or their bores. But some manufacturers may recommend a dry lube on some parts.
    I admit, I'm not familiar enough with Beretta O/U internals to know.
    But I bet if you contact Target Shotguns they will know
