Fingerprint Cards

Filling out the fingerprint cards for a form 4. What do they want for the following fields?


Employer and address (is this me, or who did the fingerprints? It's right under his/her signature.)

Reason for fingerprinting - would "ATF Form 4" suffice?

I put my actual employer information in the box and wrote "NFA Application" for reason.

i think on the back of the fingerprint card it tells you that the employer info section is for the one taking your prints

not your employer

at least thats what i have done, i have also submitted them with that blank no problems

Its for the cop fingerprinting you. That said if you leave it blank it won't make a lick of difference. Half mine have had it filled in, half have been blank and they all went thru no problem

This baffles me, according to the Instructions on FBI.gov for the form listed they are asking for the employer of the person taking the prints.?

I have a .pdf from fbi.gov that says the employer is of the person taking the prints.

Maybe a better question is:



Where do I find information for the proper completion of all parts of the fingerprint card for an NFA purchase??



I'm not sure even some of the other fields are intuitive since I have been looking around the Internet for answers.

I've always just used my personal employment information and haven't had any issues.

I asked the IMPD print folks and they told me that was what was needed.

Everything on that print card is about me - except the one place the print person signs it.

Again I have no idea if that is correct, but out of the 10+ times its been efffective 100%.

Honestly I don't think anyone every looks at that box.

Leaving it blank, putting your personal info, or the info of the person taking the prints. All are acceptable. I personally have always put the info of the person taking my prints, the local police department.

Employer and address (if applicable)

Alien Registration or INS # if Non Us. Citizen

I think the fact that the employer and address is marked with "if applicable" would suggest that this only needs to be filled in if you are applying for a job and your potential employer requires fingerprints. As far as NFA applications are concerned, this would not apply.
