    I recieved a card like this
    Emergency Medical Record
    when I took a class a while back, Good idea, I'm trying to include these cards Free to our HHRP membership. So Im looking for a hospital or facility to donate 300 of these to our club. If you know of a contact at local hospitals that would be willing to pony up some cards please let me know

    May I suggest making a template and printing them yourself for free (or nearly free) if finding a sponsor proves difficult?
    The things you'll want will be name, birthdate, allergies, medications, and history (like heart or breathing problems, seizures, diabetes, etc) and regular doctor, at a minimum.

    If you use the same pharmacy for the most part putting their contact info is a good idea also. Many times your meds change and the card does not get updated and it helps to be able to call the pharmacy and figure out what you are actually taking currently.
