Mr Ayoob had a negligent discharge with an "unloaded" revolver. It had been checked 3 times, once by him, the owner of the firearm, and the RSO. All had verified that it was unloaded. For those who say it can't happen to them. He writes about it here.

That something like that could still happen...Always being pointed in a safe direction is really your last and only real defense I guess.

Glad nobody got hurt, definitely a good reminder for prioritizing the firearms safety rules. All too often people scoff at them for being so obvious, but it only takes one screwup, and even experts like Ayoob can screwup apparently
