Can someone explain to me what mushrooming of an axe stands for? I recently found an old axe head in the barn at my family's cabin. It has a broken handle and is a little rusted but I thought I'd restore it. I have read that it should be checked for mushrooming but I can't find a definition of what that means...

Something like this, but on your Axe blade. Just file, or grind it down.

Mushrooming is normally the top of the ax, not the blade, if it has been struck to drive it through with a hammer, or has been used to break something hard with the flat side, it will deform and "pour" over the edges, normally, you would grind the edges smooth, and resurface the top to its origninal shape and reinstall a handle.
