The e-mail requirement I like but the phone field requirement seems unnecessary, but PractiScore wouldn't let Mark remove it. So you can fill it in with real data, or use gibberish, either is fine for the phone number.
You should be able to pick a squad, too. Create your own "group registration" code at the bottom of the form if you'd like.

REGISTRATION WILL RUN FROM 9:00 AM UNTIL 9:45 AM. THE 9:45 WILL BE A HARD CUTOFF, SO WE CAN HAVE TIME TO VERIFY SQUAD SIZES ETC.
