Like this recent vet complaining of stomach pains to the VA?

VA Secretary David Shulkin said Wednesday that the department is working to address the claim and prevent future mistakes. Shulkin added that the incident was "inadvertent on the surgeon's part".

Not sure if he thought anybody believed it was left there on purpose?
