    Yes on serviceable. =) If the item is in functioning condition, the answer would be yes.

    No would apply to something like a DEWAT.

    Pretty much any legit reason is good. I put "travel back and forth to XYZ state in 2016" or "Travel back and forth to family shooting range"

    Mode is whatever you're using. Personal vehicle works great.

    I've never done one of the travel forms, can one form be used for multiple trips? Your wording makes me think that one "visiting family in Pennsylvania in 2016" could cover approval for several visits. Is that how it works?

    Yes, you can file a form that covers any 365 day period. I travel to WI, MN, WA and IL with some/all of my NFA stuff. I file paperwork in October or November for each state for the next year. I had my 2016 paperwork all back approved in December of 2015. This way I'm covered.

    It'd be a total pain if you had to file for every trip. ;)

    Now, if something changed, like maybe if you had an approved form for Colorado for 2016 via personal vehicle and then a trip came up that involved a flight, I might file a separate one for that trip, but otherwise, you're covered for whatever period you put on your "From date" and "To date" up to 365 days.
