What form should I use to transfer an NFA item to the trust? Is it form 4?

If you as an individual own the item, then yes, it's a form 4.

You act as the transferor, the trust is the transferee

Yup, I own the item and need to transfer into the trust. So form 4 it is.

Yup I have my first can that needs to be put in my trust but they also want 200 bucks

For a suppressor, I don't think I would bother paying another $200 to transfer it into the trust, unless you really need other people to have posession. A machine gun might be a different story.
