    Any updates into what object pulled the trigger while the gun was in the holster? Also would there even be an investigation into exactly what happened?

    Most likely a radio antenna or long handcuff key from the belt of the officer seated next to him. There was an investigation, no one was found to have been negligent to my knowledge.

    So is it common not to find out what exactly cause the trigger to be pulled in these type of AD/ND or is it just things happen? Do they try to re-create it? I'm truly curious how this happened. I'm only asking because this is my go to holster, Safariland ALS light bearing holster. Is it normal for Safariland to get involved into something like this?

    That rig effectively covers the trigger. How would anything get down in there.

    This situation was anything but common within IMPD so there's really nothing to compare it to (we do not tolerate negligent discharges and our training and policies have historically done a phenomenal job evading them compared to agencies our size). There was no official information release on what happened that I'm aware of and if there was a recreation of the situation I've not been made aware. There were plenty of witnesses in the immediate area to confirm that no one had a pistol out of its holster and I've been told the spent casing was still in the pistol when it was removed from the holster. As I stated upthread this was a once in a thousand lifetimes event and everyone concerned appears to be satisfied with that analysis.
