Regardless of any butt-covering claim, I'm positive the FBI trains to carry with one in the chamber, and I can verify if it's a rule for you if you like. There's no assumption it's unloaded. It hits the ground, he wants to recover it as quickly as possible, reacts subconsciously, and grabs it. His finger goes in the trigger housing and fires as he grips the gun. Same reason we don't attempt to catch a falling gun, fingers can get in the trigger guard.
The lesson isn't pretend harder, it's have a plan and react consciously and deliberately. Let a falling gun fall without attempt to grab it, then pick it up administratively (vs in oh poo mode), minding muzzle direction, etc.
